Federal Tax Forms 1095-A and 8962
If you purchased a health plan in 2022 through beWellnm, you received Form 1095-A, Health Insurance Marketplace Statement. Form 1095-A provides you with information about your 2022 health coverage so you can:
- File your federal income taxes; and
- Reconcile the advanced payments of premium tax credits (APTC) you received; or
- Claim a premium tax credit (PTC) if you enrolled in a plan through beWellnm without APTC and are eligible for the PTC.
If you had coverage through beWellnm at any point during 2022, you must file your taxes and “reconcile” your premium tax credits. You’ll use the information from your Form 1095-A to fill out Form 8962, Premium Tax Credit. This is how you will find out if there’s any difference between the amount of tax credit you used and the amount you qualify for.
BeWellnm will also send your Form 1095-A to the Internal Revenue Service (IRS) to report your health insurance information for the 2022 tax year.
